FATHER IN THE NAME OF JESUS I CONFESS YOUR WORD CONCERNING HEALING AS I DO THIS, I BELIEVE AND SAY THAT YOUR WORD WILL NOT RETURN TO YOU VOID , BUT WILL ACCOMPLISH WHAT IT SAYS IT WILL THEREFORE I BELIEVE IN THE NAME OF JESUS THAT RAYMOND AND RICK ARE HEALED , ACCORDING TO { 1 PETER 2:24 }

Lord I lay this burden down at your feet, I ask that you put your loving arms around this man and heal this cancer and keep him safe. I ask that you help him to find another job or show him the way to pay for this insurance. Lord guide him in what ever needs to be your will. I ask this in your loveing name of Jesus. Lord I know that your anchor of your love will hold us and I know that the blessings will be ours if we leave it with you. Lord I am giving you this burden and leaving it at your feet. Please give me peace and understanding Lord .

Prayer Request: Please keep my husbands boss Rick in your prayers, he is going to be let go from his job on December 18th. He is a very very good friend. He has been battleing cancer for the last 6 years and will be losing his health insurance unless he pays for the expencive Cobra insurance. It saddens me that this dear sweet man is going through all of this. He has been so good to our family over the years and my heart hurts for him. I know Gos will take care of him. All the guys that have worked with him over the years are all hurting over this . Please Keep him in your prayers that he can find a new job and keep his health insurance.

Happy Thanksgiving

Raymond overall has continued to improve. His trial runs off of the ventilator have all been successful. Tuesday he did a 6 hour trial, and Wednesday all day from 6 am to 10:00 pm. The plan is to see if Raymond can be off the ventilator during the day and put back on during the night for rest and safety reasons. He did great and achieved this short term goal quickly and definitely ahead of schedule. Hope that he can keep this up. The trials off of the ventilator consist of supplying concentrated oxygen at 45% to Raymond’s trach but not any pressure or volume support he has to do all of that on his own.

Monday and Tuesday were good days for Raymond showing us that his lungs are advancing but very difficult days behaviorally. He was a complete mad man, agitated, upset and had some tears. They were very difficult days. We tried to console him many times but it was impossible. He desperately wanted his Nasogastric or NG tube out, which is used to feed him. He started another drug called the Clonidine patch to help with withdrawal symptoms to go along with Methadone and Valium, and he even got a couple of doses of Ativan. Yesterday and this today he is calmer but still not as responsive as Sunday.

Raymond is doing very well and we got new plans to layout Raymond’s immediate future for leaving the ICU and prepare him ultimately to leave Duke and arrive at Leine Children’s Hospital in Charlotte. We found yesterday that despite Raymond’s progress in his most chronic area, his lungs, that we still likely have a lengthy stay in the ICU. We still have a few issues to workout. His still has symptoms of PLE, we found out yesterday he has a deep vein thromboses (DVT) in his subclavian and axillary veins in his arm in the shoulder area, his brain bleed is only 3 weeks out and we need to address his nutrition needs going forward and he can not leave the ICU on IV continuous drip morphine, of which he has been on for 6 or 7 weeks.

The Protein Losing Enteropathy (PLE) is probably going to be the biggest issue. This is what started his sickness in early September. It is a disease that causes lose of protein or albumin in the blood stream which besides being an issue causes the side-effect of the veins leaking and causes fluid to third space around his body. The only way to get this fluid off is bring his albumin level up in his blood stream which will reverse the process and allow his veins to then draw the fluid back in to be processed by the kidneys and removed. In sever flare ups this disease can be easy to control or more difficult but is not very scientific in its approach to treatment. All of the known treatments have been discovered because someone that has PLE also was requiring treatment for other issues while usually in the ICU and it is observed to have PLE symptoms return after that treatment is stopped. The known treatments are Aldactone, Heparin, steroids, or Octreotide. Aldactone and Heparin work because they are large molecule substances that work as a sealer to prevent leakage. Steroids treat the edema that can exist in the gut wall that prevents absorption and Octreotide is a drug that was just brought yesterday that I never heard of before. One main functions of this drug is that it would help PLE is it is a vasoconstrictor which I guess have the effect of also sealing the veins because it would be denser analogous to a relaxed balloon verses a stretched one. Octreotide side effects is it inhibits secretions of many hormones including growth hormones, which Raymond’s pituitary gland already under produces that and he is on growth hormone supplements

We will not know what we are using to treat him until all three issues, recent brain bleed, PLE and DVT in his subcalavian and axillary veins are discussed with all appropriate specialist, cardiology, hematology, neural surgery and the attending ICU doctors. Each has the potential to affect the other. High dose high molecular weight heparin could be used to treat PLE and the blood clot (DVT) but would not be wise knowing we are only three weeks out from a brain bleed. We could treat his blood clot with low dose high molecular weight heparin for a few days and then switch to low molecular weight heparin (lovenox delivered by subcutaneous shots twice a day) to lower the risk of restarting the brain bleed but would not treat the PLE.

It is also recommended that Raymond get a G-tube and a central venous access before we leave the ICU. Raymond needs to be getting all of his nutrition by mouth or by G-tube before going to Levine for therapy. A G-tube is a catheter that is surgically placed in the stomach through the abdomen and taped over when not in use and that is used to put liquid food directly in his stomach. Having the NG tube slows his rehab and is a constant distraction to him.

The central venous access is a catheter in one of Raymond’s large veins that is attached to a reservoir and the entire unit is under his skin. The reservoir is next to the skin and is easily found by feeling around and allows blood draws and to give medicines without doing vein pricks. Raymond’s veins have been accessed too much over the years of surgeries and this ICU visit and need to be spared when possible. Also the ICU attending doctors don’t want to rely on getting access in an emergency if that is necessary. They emphasis that their ICU unit has highly skilled doctors and nurses to get that access under emergency conditions but that is not always the case at other facilities. Also Raymond’s needs going forward can not be easily predicted. We don’t know if he will be in and out of the hospital or if he will be followed closely in an outpatient setting.

We have been doing physical and occupational therapy for a while but will be able do more now that he is off more pain medicine and sedation. Yesterday Raymond’s arterial line finally stopped working so it was removed. It is used to get arterial blood gases analysis and to accurately display his blood pressure. It was not replaced because it is a risk of infection and they feel Raymond is stable enough now that we can monitor his blood pressure with a cuff reading. His mid pic in right arm that caused the blood clot in the subcalavian and axillary vein was removed today, to replace it he got a PIV or peripheral IV in both of his hands. Without the arterial catheter in his left foot and the mid pic in his right he can do much more movement. PT came yesterday and today to do stretching exercises. We watched closely because we like to do it at least a couple of times a day. Today Raymond participated in this therapy for the first time.

Raymond is very content today and made it all day on a trial off the ventilator and just on trach oxygen. He wants us to hold his hands and occasional will listen to us or answer a question. Helen and I gave him a sponge bath, brushed his hair and rubbed him with lotion tonight. It was great to do something so normal with him.

This has been our best Thanksgiving ever we have so much to be thankful for. Raymond’s slow road to recovery is going well and today we ate a wonderful Thanksgiving dinner at Bogart’s with our other wonderful kids and my wife’s brother and parents. The meal and company was excellent.

Thanks for praying for our wonderful family the prayers are so powerful, Raymond has a lot of fans at the hospital also, and all of his caregivers are pulling for him.

It will take some time to get through but with Prayer and supplication God will see you Through. it's different from your Adult child leaving home.it's like a knife in your heart oh Boy talking about Weeping (I did lose a couple of Pounds)

I was in a rut and Praying I couldn't understand what was going on. I was Letting Go and giving God back his child who I thought was just Mine

Jesus did, Just keep on praying because this is where your strenght will come from. keep trusting and leaning and trusting God recite PSALMS 91 the PSALMS of protection over Doug Daily and watch God . Let Go and Let God.. God will carry you through like he has done all of the Navy MOMS. It's hard and that's the Truth.
