<p>If your husband's ID has Jr on it then you need to include it on the access form. If he is not listed as a Jr on his ID (the one he wll be using) I would not included it. They used to be a little lenient on those kind of things at the gate, but they have become stricter. Names on access form should equal name on ID that will be used.</p>

<p>We did not have an Access Form at my son's PIR, but since no one has responded, I thought I would share this. Usually on forms, they offer a place for a suffix like Jr. Sr., etc, but if not, I think you put a comma after the last name and then write Jr. (last name, Jr. ) I would definitely list it and maybe even make a note if possible, especially if there is any concern between information that might be gathered for Sr. versus Jr. </p>
